  • Online: Go to the
    Registry of Motor Vehicles website
    and create a myRMV account. Provide all the necessary information. Pay the fee ($8 for an unofficial record/$20 for an attested record) and print out the driving record.
  • By mail: Print off the Public Driving Record Request Form. Make out a check or money order to MassDOT and send it with the form to:
