account, if you don't already have one. To do so, you’ll need to supply the following: Your driver’s license number
The last 4 digits of your Social Security Number
You can then go start the process of replacing your Indiana driver’s license online
. You’ll need to meet the following conditions to replace your license online: Your driver’s license is valid
You do not carry a Restriction J or a Restriction 2, 3, 4, 5, 6, 7, 8, or 9 on your driving record
Replacing a lost Indiana license in person
You may need to replace your lost or stolen Indiana driver’s license in person if the following conditions apply to you:
Your residential address, zip code, or township has changed
You changed information on your driver’s license with the BMV after receiving your last card
If you need to change your address on your driver’s license, you’ll need to visit a BMV branch in person
Remember the following when going in person to get a replacement driver’s license:
Only original documents will be accepted when proving your identity, including for your legal name, date of birth, and Social Security Number.
