Why would an at-fault party's auto insurance refuse to pay my medical costs as an injured passenger?

"I was a passenger in my friend's car and I was injured in an accident. The at-fault party's insurance said we needed to go through my friend's insurance first and they wouldn't pay my medical bills.

Is there a reason why? The other driver was at fault. "

medical payment coverage
, this would cover any of your medical expenses. If you have your own auto insurance with medical payments coverage, you would also be covered by it.
When fault is determined in an accident, the at-fault party will be liable for any injuries based on their
bodily injury liability
limits. It typically takes time for insurance companies to come to an agreement on the claim amount and what will be paid out.
The at-fault party’s insurance carrier won’t pay out for your medical bills until the accident is looked at by an adjuster and a claim agreement has been made. Medical payments coverage will provide support immediately; you don’t have to wait for a claim to be finalized.
When there is an agreement, any medical expenses that were paid out will be reimbursed by the at-fault party’s insurer. “
