You can get your Ohio driving record through the Ohio Bureau of Motor Vehicles (BMV). They keep a three-year Driving Record Abstract, which is a record of all moving violation convictions, accident involvement reports, and other actions that result in license suspensions, revocations, or other disqualifications that took place in the last three years.
There are three ways to access these records:
1. Request or view your driving record online. Simply go to the Ohio BMV’s website and log in to your OH-ID account (it’s easy to create an account if you don’t have one already). Viewing your record is free, and you can get an unofficial printed copy that includes 2 years of history for no cost. An official, certified copy will cost you $5.
2. Go to your local deputy registrar license agency. There, you’ll be able to request a certified copy for a $5 fee.
3. Mail a record request to the BMV. You can request your records by mail by completing Form BMV 1173 and paying the $5 fee. Mail both to the Ohio BMV, Attn: BMV Records at P.O. Box 16520, Columbus, Ohio 43216-6520.
