You will need to provide the following information to your local DOR office:
  • Your driver’s license number
  • Your full name
  • Your date of birth
  • Your address and zip code
You will also need to provide proof of identity and proof of Social Security.
If the information you’ve provided matches DOR documents, you’ll need to pay a duplication fee. Duplication fees differ based on your license type—capping out at $32. For example, a three-year class A license costs $26, while a six-year class B license costs $32.

What if you lose your Missouri license out of state?

To begin the process, download and print
form 4317
—the mail-in driver’s license application. Select “Replace Your Stolen or Lost MO License, Permit, or ID.” Complete the form accordingly, attaching a photocopy of the documents requested above and duplication fee payment.
Acceptable forms of payment include:
  • Personal check
  • Money order
  • Cashiers check
  • Travelers check
If you are a military member, attach a photocopy of your military ID to prove your current status. Follow the form as instructed, making sure to include “Mil” in the address.
Once completed, send your application to the address found on the form. Your replacement license will be sent by mail after its approval.
